The Community Transitional Care Team (CTCT) is a residential setting in the Downtown East Side providing a step down from acute care where DTES residents can reside while completing IV antibiotic therapy.

 

The Pennsylvania Supported Suites (PSS) is a 10 unit housing facility providing long term housing and supported care for residents.

 

Both of these programs are run as a partnership between Vancouver Coastal Health and the Portland Hotel Society, and have a diverse staff group , including nurses, program workers, social workers, peers, and physicians.

 


 

JOB SUMMARY:

Reporting to the Community Health Nurse, and under the clinical supervision of the Community Health Nurse, the Licensed Practical Nurse provides nursing care functions, such as dressings and activities of daily living.

QUALIFICATIONS:

Education, Training & Experience

  • Active registration with the College of License Practical Nurses of British Columbia is required.

 

Knowledge, Skills & Abilities

  • Proven ability to work with homeless and adults with various health/social problems.
  • Some knowledge of other cultures and lifestyles.
  • Demonstrated ability to work under pressure, to work independently with minimal direction, to multi-task, to problem solve, to take initiative, to anticipate potential problems/conflicts and take appropriate actions, to organize and prioritize work and to meet deadlines.
  • Demonstrated ability to establish/maintain satisfactory work relationships with the public, volunteers, and other employees, to deal tactfully/effectively with co-workers, public and staff from other agencies, to maintain confidentiality and to communicate effectively both verbally and in writing with this client population and the multidisciplinary team incorporating the principles of harm reduction.
  • Ability to do phlebotomies.
  • Ability to operate standard office equipment.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
