This community job will be an addition to our current Home Care Services and focus on a smooth transition for palliative patients from the hospital back into the community. The primary care nurse will be the lead in Home Care Nursing for all palliative clients, and as such is highly involved with the care provided in the community. The Primary Care Nurse will participate in care planning for the palliative patients in the community, liaise with the family physicians and other resources and do home visits on a daily basis.
The Primary Care Nurse will also work close together with the Division of Family practice to coordinate, plan and provide care for people in the community who suffer from a chronic pain condition.

Within a comprehensive care management model and within the context of a client and family centered care model and in accordance with the College of Registered Nurses’ of British Columbia (CRNBC) and/or the College of Registered Psychiatric Nurses of British Columbia (CRPNBC) standards of professional practice, Code of Ethics, the framework of departmental regulations and Community Health Nursing practice, and the established vision and values of the organization, manages a select population of eligible clients who are chronically ill or have long term physical disabilities. Uses chronic disease management, palliative and health management and/or self-care management principles to conduct assessments, identify problems, address issues or variances from the plan of care in collaboration with the appropriate physician, primary care provider, interdisciplinary team member and/or community agency. Within an interdisciplinary team context provides continuity of care across the continuum (community/acute care sites) by liaising with interdisciplinary team members, coordinating, implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care, supporting transition, and partnering with client/family/care givers to implement and maintain care and service plans. Develops and maintains partnerships with primary care providers as appropriate and in accordance with Community of Care (COC) service delivery model. Acts as a resource for the interdisciplinary team by providing information based on nursing theory and practice related to clients with complex health care needs. Consults, confers, and collaborates with multidisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in client-centred education and research.
