We currently have opportunities for experienced Community Health Nurses in our Community Programs within Vancouver Coastal Health (VCH).
Should there be insufficient qualified candidates available for the position, VCH may offer an 8-week Community Health Nurse training program – start date: April 16, 2018. Cover letter and resume are required.
Successful applicants are required to successfully complete on your own time (unpaid) the on-line prerequisite course at least one week prior to commencing the formal Community Health Nurse training program. This should only require approximately eight hours to complete. In the event you do not complete the on-line portion, then your employment offer is rescinded. Attendance is mandatory for this training/education program.
Due to the high cost of training Community Health Nurses, by accepting this training opportunity you agree to remain in the department in which you were hired for at least one year (or until the end of your temporary appointment) subsequent to completing training. This will ensure you are able to consolidate and apply your skills in the specialty area that VCH has supported and funded to complete. In the event you leave the area of specialty in under one year not covered by the Nurses’ Provincial Collective Agreement (PCA), VCH will seek to recover the total cost of the program from you in accordance with Article 35.03(E).
Works within a client and family model of care with acute, chronic and palliative populations within the community and within the context of the vision and values of the organization. The Community Health Nurse practices in accordance with the standards of professional practice and code of ethics as outlined by the College of Nurses of British Columbia (CRNBC) and standards of professional practice.
The Nurse works in partnership with the client and family to establish goals that are safe, realistic, and reasonable by assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services for the client in an interprofessional team model in collaboration with the primary care provider, and other care providers. Assesses service eligibility, facilitates the development, implementation, and adaptation of a care plan, delivers direct care, and provides care management to assist clients to manage their own care and navigate through the various services available within the community settings. The Nurse works in consultation with other system partners and incorporates current evidence into practice.
