A new, innovative, and exciting Community Health Worker position! You will support a regional ABI Transitional Housing Program that has been co-designed in collaboration with specialists from across VCH, including brain injury, rehabilitation, substance use, mental health and safety. This is the first program of its kind in BC - one that provides a healing, client-driven ("meeting the client where they are at" and "nothing about me without me"), transitional location for people after their acute brain injury and hospitalization. As part of an interdisciplinary team in a community program you will care for clients throughout their stay including transitions and outreach to the community. The ABI program also emphasizes a supportive and innovative environment to learn and expand your knowledge and expertise. As the Community Health Worker you are passionate about helping those most impacted by the housing and toxic drug supply crises and working within a team-based care environment that emphasizes trauma informed, harm reduction and culturally safe care. The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.
As per the current Public Health Orders (Long Term Care/Seniors Assisted Living Provincial Health Officer Order and the Health Sector Order), as of October 26, 2021, all employees working for Vancouver Coastal Health must be fully vaccinated for COVID-19. Proof of vaccination status will be required.
