Practices within a comprehensive care management model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for registered nurses and code of ethics, the framework of the concept of the client and family care model and the established vision and values of the organization. Provides direct health care services to marginalized women and their infants and/or children with complex needs living in the Downtown Eastside of Vancouver. Able to utilize trauma informed , harm reduction, self-care management principles to conduct assessments, identify variances, and form a plan of care in collaboration with the client, with the, interdisciplinary team member and/or community agency. Ensures continuity of care across the continuum by liaising with interdisciplinary team members, coordinating, implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care and supporting transition. Develops and maintains partnerships with primary care providers as appropriate and in accordance with the service delivery model. Acts as a resource for the team by providing information based on nursing theory and practice related to clients with complex health care needs that includes mental health and substance use issues and the impact of social determinants of health such as poverty, housing, food insecurity, personal safety and access to safe medical care. Consults, confers, and collaborates with interdisciplinary team members, educational resources and other health care professionals to achieve excellence in client-centred education and research.
About Sheway:
Sheway is a Pregnancy Outreach Program (P.O.P.) located in the Downtown Eastside of Vancouver. The program provides health and social service supports to pregnant women and women with infants under eighteen months who are dealing with drug and alcohol issues. The focus of the program is to help the women have healthy pregnancies and positive early parenting experiences.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
