As a Registered Nurse - Sheway with VCH you will:

  • Provide direct health care services to marginalized women and their infants and/or children with complex needs living in the Downtown Eastside of Vancouver.
  • Utilize trauma informed, harm reduction, self-care management principles to conduct assessments, identify variances, and form a plan of care in collaboration with the client, with the, interdisciplinary team member and/or community agency.
  • Ensure continuity of care across the continuum by liaising with interdisciplinary team members, coordinating, implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care and supporting transition.
  • Develop and maintain partnerships with primary care providers as appropriate and in accordance with the service delivery model.
  • Act as a resource for the team by providing information based on nursing theory and practice related to clients with complex health care needs that includes mental health and substance use issues and the impact of social determinants of health such as poverty, housing, food insecurity, personal safety and access to safe medical care.
  • Consult, confer, and collaborate with interdisciplinary team members, educational resources and other health care professionals to achieve excellence in client-centred education and research.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years’ recent related experience working with individuals with mental health and substance use issues, providing maternal/infant care or an equivalent combination of education, training and experience.
  • Certified practice in Contraceptive Management and Sexually Transmitted Infections.

Knowledge & Abilities

  • Knowledge of the principles and practices of woman centred care
  • Knowledge of maternal/infant/early childhood and family health and communicable disease control.
  • Knowledge of Infant feeding.
  • Demonstrated certified skills/abilities to provide contraceptive management and to assess and treat sexually transmitted infections.
  • Knowledge and skills to provide care coordination to a select caseload of clients.
  • Ability to assess clients at risk and knowledge of appropriate acts; e.g. Mental Health Act, Adult Guardianship, Public Health Act as it relates to the rights and obligations of clients and staff.
  • Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Indigenous women and children.
  • Knowledge and experience in Trauma Informed Practice, Harm Reduction and Recovery Oriented Care
  • Understands the relationship between trauma, mental health and addictions.
  • Broad knowledge of crisis intervention and counseling skills.
  • Broad knowledge of mental health issues and treatment.
  • Broad knowledge of psychopharmacology (indication and side effects).
  • Broad knowledge of substance use and treatment.
  • Broad knowledge of other facilities and community resources.
  • Demonstrated ability to conduct basic psychiatric assessments.
  • Demonstrated ability to provide treatment planning, counseling, crisis intervention, and case coordination.
  • Demonstrated ability to provide therapeutic abortion referrals and support
  • Demonstrated ability to provide nursing support in cases of sexual and/or violent assault
  • Demonstrated ability to communicate effectively, both orally and in writing, with clients and their families, colleagues, physicians, and other health care staff, both one-on-one and in groups.
  • Demonstrated listening and information seeking skills that promotes communication and lead to a cooperative approach to problem solving within an interdisciplinary setting.
  • Demonstrated ability to establish workload priorities.
  • Demonstrated ability to adjust schedule to deal with unexpected situations.
  • Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team.
  • Demonstrated ability to provide consultation and leadership.
  • Demonstrated skill in CPR techniques.
  • Demonstrated ability to problem solve.
  • Demonstrated ability to deal effectively with conflict situations.
  • Ability to operate related equipment.
  • Physical ability to perform the duties of the position.
  • Demonstrated ability to provide services through outreach in the Downtown Eastside setting such as SROs (single room occupancy housing), alleys and on the street. Collaborates with other disciplines and agencies to provide care in an outreach setting.
  • Basic computer literacy to operate a computerized client care information system and word processing, Internet and email software.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
