In the heart of Vancouver, Raven Song is committed to inclusive care for adults with mental health and substance use challenges that affect their functioning. The program is one of seven sites across the city which offers a dynamic environment for healthcare leaders. Raven Song Primary Care Clinic, located at 2450 Ontario Street in Vancouver, provides primary care services to residents of Vancouver who are living with complex clinical and psychosocial needs; are often vulnerable and under-served, and require a higher intensity of services to achieve and maintain functional stability. Clients are approached with a view to support the transition to less intense service settings if stability is gained.
This job is offered in combination with a 0.4 FTE posting at St Paul's Hospital department of psychiatry for a combined total of 0.9 FTE.
Reporting to the Medical Director, the physician who accepts this position will play a critical role in supporting the ongoing primary care needs and in the development of an inter-professional shared care team. They are responsible for providing direct clinical services to patients who present or are referred for their medical care. They foster a collaborative inter-professional practice environment that enhances client-centered services within the associated clinics/sites/programs. They work within a self-directed team of physicians and other inter-professional colleagues to achieve excellence in client-centered care.
