Reporting to the regional home health director, The CNS will provide clinical leadership and practice expertise in support of the Personalized Support and Stabilization (PSS+) team specifically and the Home Health program generally. The PSS team applies a reablement approach to care and supports transitions from acute to community for complex medical frail individuals. The CNS provides expert advise, consultation and teaching to foster and facilitate evidence based practice in the delivery of nursing care. Leads in the development of planning, development and implementation of clinical policy and practice to support ongoing program and system improvements. This leadership position is integral to supporting optimal practice in the care of complex medical and frail clients and requires knowledge and of community programming as well as skill and expertise in providing advanced nursing care for patients with acute conditions. This position has professional accountability to the VCH Professional Practice leadership team.
As a Clinical Nurse Specialist with Vancouver Coastal Health you will:
Education & Experience
Masters degree in Nursing plus current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
A minimum of five (5) year’s recent related experience working in a health care setting with a focus in Home Health and in a progressive leadership role.
Canadian Nurses Association certification in Home Health preferred.
Knowledge & Abilities
As per the current Public Health Orders (Long Term Care/Seniors Assisted Living Provincial Health Officer Order and the Health Sector Order), as of October 26, 2021, all employees working for Vancouver Coastal Health must be fully vaccinated for COVID-19. Proof of vaccination status will be required.
