Vancouver Coastal Health is looking for a Registered Nurse to join the Solid Organ Transplant team at Vancouver General Hospital. Apply today!

 

As a Registered Nurse with Vancouver Coastal Health you will:

  • Provide nursing care to assigned patients/clients working independently and as a member of an inter-professional care team.

  • Assess, plan, implement and evaluate patient/client care for the Solid Organ Transplant patient and family group.
  • Participate in discharge planning, function as a patient/client advocate and teach patients/clients and their families about illness, prevention and overall health promotion and wellness.
  • Promote the best quality of life from the perspective of the patient/client and family.
  • Document care provided and responds to incidents in accordance with established policies and procedures.
  • Consult, confer and collaborate with other health care providers.
  • Maintain and advance own clinical competence.
  • Participate on designated hospital committees/teams and approved research projects as assigned.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Skills & Abilities

  • Demonstrated ability in nursing practice related to designated patient/client group –Solid Organ Transplant.
  • Demonstrated knowledge of pathophysiology, treatment protocols, and nursing interventions related to common diagnoses and specific clinical situations related to area of practice (e.g. liver failure, renal failure, principles of hemodialysis/peritoneal dialysis, Enhanced Recovery After Surgery (ERAS) principles, kidney transplant, liver transplant, kidney/pancreas transplant, pancreas after kidney transplant, islet transplant, acute rejection, delayed graft function, maintenance for immunosuppression, pre and post biopsy monitoring, cytomegalovirus (CMV), Epstein-Barr virus (EBV), post-transplant lymphoproliferative disorder (PTLD), posterior reversible encephalopathy syndrome (PRES).
  • Demonstrated ability to provide nursing assessments & interventions related to common diagnoses and specific clinical situations relevant to the designated patient/client group (e.g. post-transplant organ specific assessments, signs and symptoms of infection and rejection, complex wound care and dressings, negative pressure wound therapy, parenteral nutrition (PN), tracheostomy care, IVIG administration).
  • Demonstrated ability to perform focused secondary assessments and integrate information from a variety of sources including diagnostics and develop plan of care relevant to the designated patient/patient/client group.
  • Demonstrated ability to anticipate and respond to potential complications and care issues relevant to the designated patient/patient/client group.
  • Demonstrated ability in correct usage of equipment and maintains competency in working with oxygen delivery equipment, central venous access devices; bladder scanners; vacuum-assisted closure (VAC) machines; other care related to tubes, catheters, drains and suction.
  • Demonstrated ability to document patient/client care on appropriate systems/forms in accordance with organizational and unit standards in a timely and accurate manner.
  • Demonstrated skill in the management of patients with acute or chronic pain and/or complications arising post-op; complex pain management issues; sensitivity to opioids or opioid tolerance. Utilizes pain and sedation assessments related to epidural, patient controlled analgesia (PCA), local anesthetic wound infusion (LAWI) & rectus sheath catheter infusion.
  • Demonstrated skill in safe care & management of oxygen therapy; chest tubes; nephrostomy tubes; urinary catheterization; bar code medication administration (various routes); performing a best possible medication history (BPMH) as part of the medication reconciliation process.
  • Demonstrated skill in assessing, maintaining, troubleshooting CVC/PICC; enteral nutrition.
  • Demonstrated skill in performing RN autonomous scope of practice and standards of care relevant to designated patient/patient/client population including IV, infection control, isolation protocols etc.
  • Demonstrated skill in educating patients around infection, rejections, incision care, self-medication at home etc.
  • General of knowledge Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.
  • Demonstrated ability to promote Indigenous Cultural Safety.
  • Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.
  • Physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
