Vancouver Coastal Health is looking for a Home Care Nurse (RN) to join the Health Service for Community Living (HSCL) team, located at G.F. Strong Rehabilitation Centre in Vancouver BC. Apply today to join our team!

 

This position supports clients with developmental disabilities throughout the city of Vancouver.

 

As a Home Care Nurse, you will:

  • Provide client care coordination including planning, delivering and evaluating the care needs of adults with developmental disability in community settings including group homes, day programs and family care homes.
  • Initiate referrals to other professionals or agencies where required, provide education and counseling to clients/families, and provide information and guidance to other caregivers and health providers.
  • Collaborate with interdisciplinary team members, with the Ministry of Children and Family Development Team and Contracted Providers, educational resources and other health care professionals/providers to achieve excellence in health services and education.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Experience working with clients with developmental disability and preferably a minimum 2 years of recent experience as a community health nurse or an equivalent combination of education, training and experience.
  • Current Cardiopulmonary Resuscitation (CPR) certificate is required.
  • Valid B.C. driver’s license. Local area travel requires the use of a personal vehicle for which mileage reimbursement will be paid.
  • An ability to speak a second language may be required.

Knowledge, Skills & Abilities

  • Working knowledge of community health.
  • Working knowledge of developmental disability and geriatric/palliative/mental health nursing practice. 
  • Working knowledge of BCCNM's standards of practice for registered nurses.
  • Working knowledge of life-span development, family theory, multicultural health practices, principles of health education/promotion, and determinants of health.
  • Demonstrated abilities in facilitation, mediation, and conflict resolution techniques, and demonstrated analytical and problem solving skills.
  • Demonstrated abilities in client advocacy.
  • Demonstrated discharge planning and/or case coordination.
  • Demonstrated ability in conducting physical/mental/psychosocial/functional and environmental assessments.
  • Demonstrated ability to prepare and maintain a variety of charts, records and reports related to work.
  • Demonstrated ability to independently plan, organize and prioritize workload and adapt to a changing caseload.
  • Demonstrated ability to plan/deliver in-services to individuals and groups.
  • Demonstrated ability to communicate effectively, both verbally and in writing.
  • Ability to establish and maintain effective working relationships.
  • Ability to work effectively in an interdisciplinary team environment and on committees with key stakeholders.
  • An ability to critically read and utilize current health research.
  • Basic computer literacy.
  • Ability to carry out the physical demands of the job.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
