As a LPN Coordinator at VCH you will:

  • Act collaboratively as a member of an interdisciplinary care team to perform full scope practical nursing functions as outlined by BCCNM for Licensed Practical Nurses.
  • Provide direct nursing care by assessing and admitting clients, planning, implementing, evaluating care plans, and maintaining the health records of clients.
  • Provide leadership and clinical support to the multi-disciplinary team by coordinating client care activities and providing supervision to staff that provide Adult Day Care Services.
  • Consult, coordinate and collaborate with multidisciplinary team members, educational resources and other health care professionals/providers to achieve excellence in client care.
  • Assist with administrative activities to provide optimal use of health care resources.

Qualifications

Education & Experience

  • Graduate of a recognized program for Licensed Practical Nurses. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Current Medication Certificate.
  • Three (3) years' recent related experience in gerontological nursing that includes (2) years recent supervisory experience, or an equivalent combination of training, education and experience.

Knowledge, Skills & Abilities

  • Knowledge of nursing theory and practice within a member and family centred model of care.
  • Knowledge and practice within the standards dictated by the BCCNM for Licensed Practical Nurses.
  • Knowledge of other health care disciplines and their role in patient care.
  • Demonstrated ability to provide leadership, work direction, supervision, and to coach team members to promote an efficient and effective team based environment.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Demonstrated ability to communicate with and deal effectively with clients and their families, co-workers, physicians, other health care staff and staff of outside agencies.
  • Demonstrated ability to adjust to new or unexpected events.
  • Demonstrated ability to deal with conflict situations.
  • Demonstrated ability to communicate orally and in writing.
  • Demonstrated ability to plan, organize and prioritize work.
  • Demonstrated physical ability to perform the duties of the position.
  • Demonstrated Skill in CPR. 
  • Basic computer literacy to operate a computerized client care information system.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
