Come work as a General Practitioner with Vancouver Coastal Health (VCH)!

The physician working in this program will collaborate with a multidisciplinary team of health care professionals to serve a vulnerable population. The site provides a patient-centred, geriatric-focused environment where assessments and management can be done thoroughly and progress can be monitored. These patients are under the care of the GP MRP and have the availability of consulting Geriatric Psychi patients on your panel

  • Work with Specialists and Interdisciplinary team to optimally assess and treat patients.
  • Collaborate w patient’s primary care Provider
  • Follow the complex medical issues and medication regimes of referred patients, in collaboration with their family physician over a period of approx 20 visits
  • Involvement in interdisciplinary team rounds, care conferences and family meetings as needed
  • Involvement in intake meetings and in the triage process as needed
  • Liaison with community primary care providers, involved specialists, and pharmacists to facilitate safe, optimal discharges
  • Liaison between Inpatient Unit and site for appropriate patients who transition from one to the other
  • Expectation to be available outside of sessional hours for urgent lab or diagnostic results during daytime weekday hours (this may shared among other DH GPs)
  • Evening or weekend on call is 1 in 9 and covered by MOCAP funding

Qualifications

Education, Licensing, & Experience:

  • Full or provisional licensure by the BC College of Physicians and Surgeons
  • Current College of Physicians of BC (CPSBC) – Full
  • Canadian Medical Protective Association (CMPA) - Mandatory
  • Experience in elder care

He or she will report to the Program Medical Director, Community Geriatric Health Services, the STAT Centre Medical Coordinator, and through them to the Senior Medical Director, Vancouver Community. All VCH physicians must be appointed as medical staff of Vancouver Coastal Health. A clinical faculty appointment with the University of British Columbia Faculty of Medicine is desired as resident supervision and involvement in education initiatives is an expectation.
