As an LPN at Vancouver Coastal Health, you will:

  • Perform, implement, plan and evaluate nursing care for work assignments at assigned locations within the competency and practice guidelines established by the British Columbia College of Nurses and Midwives (BCCNM), legislative scope of practice and consistent with Vancouver Coastal Health policies and procedures.
  • Act as a liaison between the acute care facility and community care service to facilitate discharge planning from the acute care facility to the appropriate service in the community.
  • Enhance quality of life from the perspective of the client/family.
  • Establish, maintain, and enhance therapeutic relationships based on respect.
  • Provide care to clients and families based on stable and predictable states of health by initiating and following established plans of care.
  • Recognizes and responds to unexpected client outcomes by communicating and collaborating with other members of the team.
  • Document care provided and respond to incidents in accordance with established policies and procedures.
  • Consult, confer, and collaborate with other health care providers as needed.

Qualifications

Education & Experience

  • Graduation from an approved program of Practical Nursing, plus one (1) year recent related acute care, residential and/or community-based clinical experience, or an equivalent combination of education, training and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes, as required.

Skills & Abilities

  • Demonstrated ability in nursing practice related to designated client group. 
  • Demonstrated knowledge of pathophysiology related to common diagnosis related to designated client group. 
  • Demonstrated ability to perform nursing assessments and interventions related to common diagnosis and specific clinical situations relevant to the designated client group including safe medication practices.
  • Demonstrated ability to make sound clinical judgements to provide safe ethical client care to manage a select population of clients. Has knowledge of and accesses appropriate resources embedded within the service delivery model to support nursing practice. Knows when and how to seek out clinical support and guidance as needed.
  • Demonstrated ability to recognize potential complications and care issues relevant to the designated client group by alerting the RN and/or most responsible provider.
  • Demonstrated ability in correct usage of equipment and maintains competency in working with, for example, oxygen delivery equipment; vital signs monitor; glucometer; IV pumps; splints, dressings and walking aids and other care related equipment.
  • Demonstrated ability to document client care on appropriate systems/forms in accordance with organizational and unit standards in a timely and accurate manner.
  • Demonstrated ability to provide safe and appropriate telephone consultations and document related discussions and client care on appropriate systems/forms in accordance with organizational and unit protocol.
  • Demonstrated skills in patient/family centered care planning, patient/family engagement and education; and therapeutic communication.
  • Demonstrated skill in performing LPN scope of practice and standards of care relevant to designated client population including infection control, etc.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
