As a Team Leader, Primary Care with VCH you will:

  • Oversee and coordinate the delivery/daily activities of designated programs/areas.
  • Evaluate services to ensure program/service needs are met and liaises with key partners on the achievement of program goals and objectives.
  • Promote interdisciplinary practice and fosters a safe environment for staff and clients.
  • Provide leadership for the delivery of a service, working in partnership with the organizational resource areas, including Professional Practice, Clinical Education, Clinical Quality and Safety, and other organizational key partners, to ensure a high standard of care founded in best practice and standards of care.
  • Provide supervision, to at least one (1) other health science professional and a total staff of more than eight (8) up to and including sixteen (16) FTE which may include Concurrent Disorders Clinicians, Clinical Counselors, Care Coordinators and/or support staff such as Community Liaison Workers and Clinical Support Attendants.
  • Provide continuous communications, conflict resolution, performance recognition, coaching, performance management, safe work environment, facilitates client transitions, and manages workload and staffing issues.
  • Support the Manager with programs related to attendance wellness promotion, respectful workplace, accommodation, recruitment, hiring, selecting, mentoring and retaining staff.
  • Maintain a clinical caseload, plan and implement social work services by conducting social and psychosocial assessments;
  • Participate with clients, family, community support and interdisciplinary team in case management meetings and maintaining related client records.

Qualifications

Education & Experience

  • Current full registration with the British Columbia College of Social Workers.
  • Master of Social Work from an accredited School of Social Work.
  • Five (5) years recent related clinical experience working with individuals with mental health and/or primary care needs including two (2) years supervisory and/or leadership experience or equivalent combination of education, training and experience.
  • Valid B.C. Driver's license and local area travel requiring the use of a vehicle may be required.

Knowledge, Skills & Abilities

  • Broad knowledge of systems thinking as it related to social work process.
  • Broad knowledge of other health disciplines and their role in healthcare.
  • Broad knowledge of case management and practice within a client and family centred care model for clients with complex health care needs.
  • Knowledge of appropriate acts; e.g. Mental Health Act, Adult Guardianship, Public Health Act as it relates to the rights and obligations of clients and staff.
  • Broad knowledge of counselling skills and clinical practice models.
  • Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations
  • Knowledge of cultural diversity in youth and families, and culturally safe ways of engaging.
  • Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention.
  • Demonstrated ability to conduct social and psychosocial assessments.
  • Demonstrated ability to understand and influence group and inter-group processes, decision-making and problem solving.
  • Demonstrated ability to establish rapport with clients and families in order to explore issues that are beyond the initial presented concerns.
  • Demonstrated ability to plan, implement, facilitate and evaluate group programs, as applicable.
  • Demonstrated ability to function as an effective team leader, motivator, coach/mentor, and team member within a multidisciplinary setting.
  • Demonstrated ability to model and foster a positive, collaborative, and respectful team working environment.
  • Demonstrated ability to provide clinical and administrative supervision and consultation.
  • Demonstrated skills and ability to facilitate team building and to work both independently and collaboratively.
  • Demonstrated ability to adjust to new or unexpected events, problem solve, and deal effectively with and/or guide others in resolution of conflict issues.
  • Demonstrated ability to communicate effectively, both orally and in writing, with clients and their families, colleagues, physician, and other health care staff, both one-on-one and in groups.
  • Demonstrated listening and information seeking skills that promotes open and direct communication and leads to a cooperative approach to problem solving.
  • Demonstrated ability to set priorities, organize work, and meet deadlines.
  • Demonstrated ability to work effectively with community partners and with individuals from diverse backgrounds.

  • Physical ability to perform the duties of the position.
  • Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
