Vancouver Coastal Health is looking for a Social Worker to join the Mobile De-Escalation Team team in Vancouver, BC. Apply today to join our team!

 

As a Social Worker with Vancouver Coastal Health, you will:

  • Be responsible for providing clinical and consultative social work services of a general nature to clients and families.
  • Perform duties including:
    • conducting social assessments
    • assisting clients with system navigation and practical needs related to social determinants of health
    • maintaining records
    • attending interdisciplinary clinical/case conferences
    • identifying applicable community resources for clients and facilitating care throughout the continuum

 

About Mobile Crisis De-Escalation (MoDe):


This new service will provide mobile crisis response to individuals in the inner city who are experiencing a mental health or substance use crisis. The interdisciplinary team will work closely with community service providers including shelter and supportive housing staff to provide a community-based, healthcare-led crisis response, post-crisis follow up, and service navigation. This is an exciting opportunity to be part of a new, specialized service that will evolve to meet the needs of the inner city community.

 

Qualifications

Education & Experience

  • Current full registration with the British Columbia College of Social Workers.
  • Bachelor’s degree in Social Work from an approved school of Social Work.
  • Two (2) years’ recent related experience working with marginalized clients with multiple health issues (i.e. HIV and mental health and substance use issues) in community based or Indigenous-led or First Nations organizations, or an equivalent combination of education, training and experience.
  • Valid BC Driver’s License and access to a personal vehicle for business-related purposes.

Knowledge & Abilities

  • Basic knowledge of systems thinking as it relates to social work process.
  • Working knowledge of counselling skills.
  • Understanding of local (i.e. Downtown Eastside) and other regional initiatives that may affect patient care, resource access or practice guidelines in this local clinic setting.
  • General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.
  • Demonstrated ability to promote Indigenous Cultural Safety.
  • Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations
  • Working knowledge of other health disciplines and their role in healthcare.
  • Working knowledge of legislation and government policies and procedures that relate to the client.
  • Demonstrated ability to conduct psychosocial assessments.
  • Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention.
  • Demonstrated ability to draw out and honour information that will contribute to understanding the clients’ situation.
  • Demonstrated ability to communicate effectively both verbally and in writing with clients, families, team members and community agency personnel.
  • Demonstrated ability to adjust to new or unexpected events.
  • Demonstrated ability to deal effectively with conflict situations.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Ability to operate related equipment including applicable software applications.
  • Physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
