Vancouver Coastal Health is looking for a Registered Nurse to join the Transition Services team at Lions Gate Hospital in North Vancouver, BC.  Apply today to join our team!

 

As a Transition Nurse with VCH, you will:

  • Receive and/or prioritize referrals for patients who require home and community care services or transitional care/services such as home care nursing, home support services, rehabilitation therapy, case management, assisted living/residential care.
  • Work collaboratively with the interdisciplinary team to provide continuity of care across the continuum by assessing patients, liaising with team members, coordinating, implementing discharge plans, evaluating patient outcomes based on patient goals for care and supporting transition to and from community and facility settings.
  • Establish, maintain and enhance therapeutic relationships based on respect.
  • Support patients and families to achieve and maintain optimal health by performing the following duties and responsibilities.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years’ recent related clinical nursing experience in an acute/community care setting working with a variety of patient populations, or an equivalent combination of education, training and experience.

Knowledge, Skills & Abilities

  • Comprehensive knowledge of BCCNM standards of practice and guidelines for registered nurses.
  • Comprehensive knowledge and demonstrated ability to apply intensive case management principles within a patient and family/care giver centred care model for patients with complex health care needs.
  • Knowledge of indigenous cultural safety, culturally safe principles and practices of person and family centered care, trauma-informed care, and harm reduction.
  • Comprehensive knowledge and demonstrated ability to apply chronic disease and/or primary care management and health promotion principles.
  • Demonstrated ability to problem solve using sound judgment in applying critical thinking skills within safe limits of patient care.
  • Ability to engage in culturally competent and responsive care.
  • Demonstrated skill in physical assessment and chronic disease management.
  • Competency in psychomotor skills within the designated scope of nursing practice.
  • Ability to work independently and set care priorities for a patient caseload.
  • Knowledge of other health disciplines and their role in patient care and other health and community resources related to the care of patients and caregivers.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Ability to establish workload priorities in collaboration with others.
  • Knowledge of adult education principles, methods and tools.
  • Ability to use research and methodology in practice.
  • Ability to provide leadership and work direction.
  • Ability to teach, coach and facilitate patients and families/care givers individually or in group settings.
  • Demonstrated ability to effectively communicate both verbally and in writing.
  • Effectively communicate and collaborate with negotiate with co-workers, physicians, other health care staff, patients, families/care givers and staff of outside agencies.
  • Effective conflict resolution skills.
  • Ability to adjust to new or unexpected events.
  • Ability to conduct physical, mental, psychosocial/functional environmental assessments.
  • Ability to use of supplies and materials utilized in the provision of community based nursing service.
  • Ability to engage in self-reflection for the purpose of professional growth and development.
  • Ability to operate related equipment including applicable computer software applications.
  • Physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.

Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
