ABOUT THE HOME-VIVE PROGRAM
Home-ViVE is an integrated, proactive program providing longitudinal primary care, nursing and rehabilitation services to frail homebound seniors in Vancouver.
Our goals are to support clients and family/caregivers who wish to remain in their home for as long as possible, and to assist them to avoid unnecessary emergency room visits or hospital admissions.
The Home-ViVE team will work very closely with clinicians in the Community Health Centres. Most clients will have a case manager in a Community Health Centre who will remain involved with the client. Clients may also have intermittent involvement with other community clinicians. All clients will be assigned a Family Physician or Nurse Practitioner who will provide primary care services in the home (with 24/7 on call MD coverage). Episodic, short-term, intensive nursing and rehabilitation support will be provided by the Home-ViVE team. The Home-Vive RN, PT, OT and Rehabilitation Assistant may actively collaborate and communicate with the case manager and other Community Health Centre clinicians. For Home-ViVE clients who are admitted to an acute care unit, the Home-ViVE team will be able to assist with collateral information and participate in discharge planning, with the goal of providing clinical supports to allow the client to return home as soon as safely possible.
