Vancouver Coastal Health is looking for a Support Worker to join the Robert and Lily Lee Family Community Health Centre (CHC) in Vancouver, BC. Apply today to join our team!

 

As a Support Worker with VCH you will:

  • Work as part of an multidisciplinary pre-natal outreach program to provide supportive counselling, food access, and health promotion education to support a specific/designated population group of pregnant women of all ages to have healthy pregnancies.
  • Conduct interviews to obtain information such as social risk factors and client centre goals, collaborates with team, prenatal assessment according to established guidelines, assisting with developing and revising care plans, establishing and maintaining relationships based on respect with participants, and provides culturally sensitive health promotion education, guidance and supportive counselling.
  • Provides referrals for community/social supports; plans, arranges and co-facilitates drop-in groups; liaises with medical community, hospital and community agencies ; maintains clients records; organizes food buying club and Community Kitchens; and purchases, prepares, portions, and assembles food items according to food safe protocols.

Qualifications

Education & Experience

  • Grade 12 supplemented with courses in social services, nutrition, counselling or health sciences at the College level and two (2) years’ recent related experience in community outreach, group facilitation and supportive counselling with designated immigrants, preferably with pregnant and parenting women/families, or an equivalent combination of education, training or experience.
  • Ability to speak a second language is required.
  • Food Safe Certificate.
  • Valid BC Drivers License. Local area travel may require the use of a personal vehicle.

Knowledge & Abilities

  • Comprehensive knowledge of designated culture and appropriate community resources and agencies.
  • Ability to communicate verbally and in writing.
  • Broad knowledge of prenatal nutrition, food security, prenatal health and breastfeeding.
  • Knowledge of smoking cessation and addictions.
  • Knowledge of infant nutrition, growth and development.
  • Skilled in group facilitation and supportive counselling.
  • Demonstrated ability to establish and maintain rapport with clients, coach group participants.
  • Ability to network within the designated community environment.
  • Ability to communicate effectively and proficiently both verbally and in writing.
  • Ability to deliver culturally sensitive services.
  • Ability to establish and maintain rapport with clients.
  • Ability to work independently and in cooperation with others.
  • Ability to organize and prioritize.
  • Ability to analyze and resolve problems.
  • Physical ability to carry out the duties of the position.
  • Ability to operate related equipment.
  • Basic computer literacy to operate computerized information system and word processing, spreadsheet, Internet and e-mail software.

Closing Statement

Language skill in Arabic would be an asset.

 

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.

As per the current Public Health Orders, as of October 5, 2023, all employees working for Vancouver Coastal Health must be fully vaccinated for COVID-19 or have received a single dose of the most-recent, updated COVID-19 vaccine. Proof of vaccination status will be required.
