Vancouver Coastal Health is looking for a Registered Nurse (RN) to join the Chronic Disease Services team at West Vancouver Community Centre in West Vancouver, BC. Apply today to join our team!

 

As an Chronic Disease Prevention Management Nurse with Vancouver Coastal Health you will:

  • Provide chronic disease management education and clinical intervention to a designated client population on an individual and/or group basis who are at risk for chronic diseases.
  • Participate in the development, implementation, delivery and evaluation of education and early intervention and chronic disease management programs.
  • Conduct outpatient assessments.
  • Support client self-management of chronic disease through teaching, counselling and lifestyle adjustments.
  • Share information and collaborate with other healthcare teams to ensure the provision of quality chronic disease management services through the healthcare system.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years’ recent related clinical experience in a community and/or outpatient care setting assessing and treating clients with chronic diseases such as diabetes within a chronic management framework including supporting self management, or an equivalent combination of education, training and experience.
  • Eligible for accreditation as a Certified Diabetes Educator (CDE).
  • Valid BC Driver’s license and access to a personal vehicle for business-related purposes, as required.

Knowledge, Skills & Abilities

  • Comprehensive knowledge of nursing theory and practice within a client and family centred model of care.
  • Comprehensive knowledge of the BCCNM Standards of Practice for registered nurses.
  • Comprehensive knowledge of evidence based nursing practice related to diabetes (prevention/management) and diabetes education.
  • Comprehensive knowledge of chronic disease management and self-management support.
  • Working knowledge of community resources.
  • Comprehensive knowledge of other health care disciplines and their role in client care.
  • Comprehensive knowledge of cultural diversity and its relation to healthy lifestyle behaviors.
  • Broad knowledge of research process and methodology.
  • Demonstrated ability to provide leadership and work direction.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Demonstrated ability to communicate orally and in writing.
  • Demonstrated ability to develop and present educational programs and materials.
  • Demonstrated ability to communicate effectively, both verbally and in writing with co-workers, physicians, other health care staff, clients and their families and outside agencies.
  • Demonstrated ability to adjust to new or unexpected events.
  • Demonstrated ability to plan, organize and prioritize work.
  • Demonstrated ability to deal with and/or guide others in resolution of conflict issues.
  • Demonstrated skill in the use of medical equipment and supplies appropriate to Diabetes.
  • Demonstrated skill in techniques appropriate to Diabetes Education.
  • Demonstrated skill in CPR techniques.
  • Demonstrated physical ability to perform the duties of the position.
  • Demonstrated ability to access computerized multi-media resources for diabetes education.
  • Demonstrated ability to organize, analyse and problem solve.
  • Ability to operate related equipment including applicable software applications.
  • Physical ability to perform the duties of the position.

Closing Statement

Position is with the North Shore Diabetes Education Centre. Clinical experience required with clients who range in age from Pediatric to Geriatric in the prevention and/or management of Type 1, Type 2 and Gestational Diabetes. Education and care is offered as in-person and/or virtual group and individual 1:1 appointments. 
 
Knowledge/experience supporting clients on insulin pump therapy and CDE certification are assets.
