Acute Home Based Treatment Team

 

Join a specialized community mental health and substance use team targeting the client’s journey as a crucial transition point in their continuum of care.

 

Our program will provide short term, crucial in-home acute psychiatric care for clients 17 & up, who require immediate and intensive support as an alternative to hospital. The client will have to consent to treatment within their home environment.

 

Clinical staff will utilize their knowledge of the full spectrum of mental health disorders, connect and assist with navigation of Vancouver Community Mental Health and Substance Use (MHSU) services. Staff will play a key role for the client and to healthcare providers in the system. The team will support the client from hospital back home and will also provide treatment at home as an alternative to hospitalization when appropriate.

The team is active and passionate about their work in mental health and substance use. The acute and community systems will rely on this team to do the very important work of providing comprehensive mental health and substance use care in the client’s home. There will be strong clinical support and leadership to ensure this team remains innovative and productive. All clinical staff will feel competent that they’re contributing to improving mental health and substance use care of the clients living across Vancouver.

 

As this is a city-wide team, we will see a wide range of clients across all socio-economic backgrounds. We will have close relationships with Vancouver General Hospital and St. Paul’s Hospital. In the community we will also work closely with all Vancouver Coastal Health community MHSU teams and be familiar with other outreach-only teams.

As a Care Coordinator with the Acute Home-Based Treatment team with VCH, you will:

  • Work as part of a multidisciplinary community team using a strengths-based model, comprehensive treatment plans and discharge planning services to support clients dually diagnosed with mental health and addictions.
  • Provide treatment services including crisis intervention and group facilitation.
  • Establish and maintain relationships with clients.
  • Collaborate with multidisciplinary team and other community resources, liaise and consult with internal/external physicians, governmental and community social service, health and mental health agencies in facilitating clients’ access to services.
  • Participate in committees and meetings, engages in on-going team planning, development and quality improvement activities, and maintain and update your own knowledge.

Qualifications

Education & Experience

  • Bachelor’s degree in a behavioural science from a recognized program.
  • Two (2) years’ recent related experience or an equivalent combination of education, training or experience.
  • Valid BC Drivers License. Local area travel may require the use of a personal vehicle.

 

Knowledge & Abilities

  • Knowledge of the principles of recovery in mental illness.
  • Broad knowledge of drug and alcohol addiction issues, treatment including non-violent intervention, and the recovery process.
  • Broad knowledge of psychotherapeutic and counselling skills.
  • Broad knowledge of other facilities and community resources.
  • Broad knowledge of public housing and housing issues, both locally and provincially.
  • Broad knowledge of legislation and government policies and procedures that relate to the client.
  • Demonstrated ability to conduct social, psychosocial and substance abuse assessments.
  • Demonstrated ability to communicate effectively, both orally and in writing, with clients, colleagues, physicians, and other health care staff, both one-on-one and in groups.
  • Demonstrated ability to apply knowledge of theory and practice to a case management process.
  • Demonstrated ability to establish rapport with multi-barrier clients.
  • Demonstrated ability to build and maintain relationships with a variety of individuals/service providers.
  • Demonstrated ability to work both independently and collaboratively as a member of an interdisciplinary team.
  • Demonstrated ability to adjust to unexpected events, analyze and problem-solve, set priorities, deal effectively with conflict, and respond to crisis.
  • Physical ability to perform the duties of the position.
  • Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.  Successful applicants may be required to complete a Criminal Records Review Check.

As per the current Public Health Orders, as of October 5, 2023, all employees working for Vancouver Coastal Health must be fully vaccinated for COVID-19 or have received a single dose of the most-recent, updated COVID-19 vaccine. Proof of vaccination status will be required.
