ABOUT SUMAC PLACE:
Sumac Place is a 28-bed specialized mental health facility located in Gibsons, B.C. It is part of Vancouver Coastal Health’s plan to meet the need for specialized mental health services throughout the region and serves clients from the Sunshine Coast as well as from other locations in the region. Sumac Place serves adult patients with complex mental illness who need further supports in order to optimize their independence and quality of life. Clients are stable and require specialized rehabilitation services as they continue on their recovery journey.

JOB SUMMARY:
Within the context of a client and family-centred Recovery model, and in accordance with established vision and values of the organization, the Social Worker 2 works without general supervision as a member of an interdisciplinary mental health team. Conducts comprehensive social and psycho-social assessments. Collaborates with the client and interdisciplinary team to facilitate transitions through the continuum of services for clients with complex discharge and/or intake/referral needs. Provides individual, family and group counselling services. Attends clinical/case conferences as a member of an interdisciplinary team for client reviews, admissions and discharges. Provides consultative and direct services to clients, families, staff and community health professionals regarding issues such as adult guardianship, advanced directives, domestic violence, abuse and/or neglect, immigration, housing, financial concerns, or matters concerning government agencies (public trustee) and authorities (courts, police). Prepares and maintains client records in accordance with established standards, policies and procedures.
